PLACES is a map tool that integrates data from the CDC into a map of the United States.
Select a pieces of data at the top of the page. You can look at health data in the three categories of Health Outcomes, Prevention, or Unhealthy Behaviors at the very top of the page and then select a piece of data from that category just underneath.
Once you've selected a piece of data you're interested in, you can use the magnifying glass icon on the top right of the page to search for an address, like a hospital or a university. Click on the area and use the pop up window's left and right arrows to see the data for all of the county and for that specific area's census tract.
